Assessments

Often we will use standardized clinical assessments to help you and us determine the best way to help you. If so we will notify you and assign you a "Client Identifier"

To take an assigned assessment, follow these instructions:

Go to new.recoveryzone.com
Click on Client Access
Enter the therapist's name by typing: Restored Mentoring and then selecting our name from the drop-down list
Enter the Assigned Identifier given to you by us.
Click Log In and take the assessment
Some assessments are lengthy (up to 2 hours). You can save your work and return later if needed for longer surveys
Notify us once you are finished so we can begin looking for the results.
We will review the results with you in about 2 weeks.
